If you need to take a break and come back to complete your questionnaire at a later time, you will need this number to retrieve your responses so far. If you need to return to continue your questionnaire, you only need to enter your First Name, Last Name, Phone Number, and Trainer Code. You may leave the other fields blank. You will need to click through each page using the "Next Page" button, until you get to the page you want to continue from. You may change any of your previous answers or you may begin where you last left off.
